Part-Time Case Manager
New Beginnings Counseling Center is seeking a part-time Case Manager for its Supportive Services Case Management contract with the Housing Authority of the City of Santa Barbara. Position is a 25 hours-per-week part-time position, with an hourly rate of $20.00.
The position performs case management duties involving Housing Authority families, seniors, disabled and/or formerly homeless clients that will foster their participation in various Housing Authority programs, as well as programs and services offered by various local agencies designed to achieve self-sufficiency and stability. A Master’s degree or higher in Social Work, Marriage and Family Therapy, or the related field-work equivalent, is highly preferred, with experience working with the homeless population, community mental health or related field a plus. Intern hours and supervision available.
Position requires the ability to empathize with and have the ability to advocate for low-income and vulnerable people, and to maintain confidentiality with personal information. Knowledge of social services and local resources with the ability to provide referral information to meet individual and family needs is a plus. Must be able to communicate effectively both written and orally. Must be a self-starter and be self-motivated and work with a minimum of supervision. Should have excellent interpersonal skills, with emphasis on effectively working in a team environment and with a diverse population. It is important for candidates to also have appropriate and healthy boundaries and a working knowledge of the legal and ethical guidelines concerning the mental health professions.
- Meet with clients, family members and other interested parties to assess clients’ needs for social services;
- gather and evaluate information regarding client history, housing situation, physical functioning, financial status, and capacity for independent living;
- evaluate clients’ concerns and observe behaviors;
- develop service plans; and maintain case files;
- work closely with landlords to build relationships and house hard-to-place individuals and families;
- provide prevention services, crisis management, referrals to shelter and transitional housing, permanent supportive housing and aftercare/follow up;
- counsel clients on available resources, barriers to employment, independent living skills and other areas involving defined problems or concerns;
- explain procedures, rights and responsibilities about available services;
- assist clients in identifying and obtaining basic services needed for independent living;
- identify and make referrals to a variety of public and community agencies providing medical, educational, housing, and other services;
- schedule client’s appointments; act as client advocate in obtaining services. Manage assigned caseloads; prepare reports, documents and correspondence regarding client status; document case files;
- develop service treatment plans, evaluate behavioral adjustments and monitor client progress toward delineated objectives;
- counsel clients using a variety of counseling modalities;
- represent New Beginnings Counseling Center with tact and diplomacy and ethical behavior; and
- participate in staff meetings.
In-home client service provision, independent local automobile travel, and occasional client transportation in Housing Authority vehicles is required. Mileage reimbursement is provided when using own vehicle.
Somewhat flexible work hours are available during the traditional work week of Monday – Friday, 8-6. Position begins asap.
